SWOC-S EXAMINATION INFORMATION & PRICING
Exam Pricing: $600
Interested Advanced Level Providers (Medical Doctors (MDs), Doctors of Osteopathic Medicine (DOs), Physician Assistants (PAs) and Nurse Practitioners (NPs)) should call to register for examination and submit all supporting documentation, which includes:

Copy of Driver’s License or Other Government Issued Identification

Copy of Professional License

Evidence of 14 contact hours of education in Skin, Wound, and Ostomy education by any program that is credentialed by the American Medical Association.
